3. Fair Participation Forms

  Eligible participants must submit this form if they are showing animals in the County Bred and Fed contest.

How to Submit Forms

Online Submission:
Some forms may be submitted directly online. Click the links to access digital forms where available.

In-Person Submission:
Drop off your completed forms at the San Miguel Basin Extension Office located at 1120 Summit Street, Norwood, CO.

Mail Submission:
Mail forms to the San Miguel Basin Extension Office, P.O. box 130, Norwood, CO. Please allow sufficient time for delivery before deadlines.

Email Submission:
Scan and email your forms to Annika.
