How to Submit Forms
Online Submission:

In-Person Submission:
Drop off your completed forms at the San Miguel Basin Extension Office located at 1120 Summit Street, Norwood, CO.
Mail Submission:
Mail forms to the San Miguel Basin Extension Office, P.O. box 130, Norwood, CO. Please allow sufficient time for delivery before deadlines.
Email Submission:
Scan and email your forms to Annika.
